To obtain a copy of your child’s vaccination history, please place this in your cart and check out. Please note, this form is NOT sufficient for school/preschool/daycare. For school required forms, please see School Forms (DH3040 and DH680).
With your permission, we can email the completed form(s) to you or you may elect to pick up the original forms in-person at our office. Typical processing time may take as long as 4 to 5 business days.
